About Care Atlas

Care Atlas is a free, independent directory of Medicare-certified nursing homes, home health agencies, hospice providers, hospitals, dialysis centers and rehabilitation facilities in the United States. We bring together the official quality data that the Centers for Medicare & Medicaid Services (CMS) publishes – star ratings, inspection citations, staffing, fines and ownership – and present it in one place, city by city.

Where the data comes from

All facility information comes from the CMS Provider Data Catalog (Care Compare), which CMS updates monthly. We import each release and show the data as published. We do not collect reviews and we do not accept payment from facilities for placement or ratings.

Care Atlas is a project of service-lab.site and is not affiliated with, endorsed by or operated by CMS, Medicare or any government agency.
