Every Medicare- or Medicaid-certified nursing home is inspected about once a year and rated by the Centers for Medicare & Medicaid Services on a 1–5 star scale for health inspections, staffing and quality of care. We show those ratings together with the underlying inspection citations, fines, staffing hours and ownership so you can compare facilities side by side.
Ratings are a starting point, not a verdict. Visit in person, ask about staffing on weekends and nights, and read the most recent inspection report before you decide. How CMS ratings work →
